California Success Story
The Population Health Management Initiative (PHMI) is a collaboration between the California Department of Health Care Services, Kaiser Permanente, and Community Health Centers. For this project, Innovaccer’s GHAAP creates a foundational data layer that powers population health management and care management workflows, aggregates data to provide 360-degree patient views, and augments physician engagement processes. It enables Community Health Centers to strengthen their ability to provide equitable health care services, as well as drive improvements to produce equitable health outcomes. Stakeholders receive actionable patient insights for more informed decision-making.
