The GTS VirtualHealth Platform (GTS-VP) is the remote patient monitoring platform designed to address the growing and changing needs of healthcare to move from local care to treatment at distance.

Because we are only just beginning to understand what is possible using RPM, the underlying premise of any enterprise RPM solution must be the ability to provide a robust capability for today, but also be designed to integrate and implement capabilities to do not yet exist.

  • GTS VirtualHealth – Patients access the GTS-VP through a mobile app that is freely available through the Apple Store and Google Play
    • Easy to Use: If Patients won’t use the tools, there is no way to get better health outcomes
      • Simple – In order to ensure the greatest possible adoption by patients, the app must be as simple as possible, but still allow for a range of activities to be performed.
      • Dynamic - The app adapts to show only those capabilities that are required for the patient and it changes as the orders given to the Patient change.
    • Wide range of information collection:
      • Vital Readings – The GTS-VP is designed to work with a wide range of medical devices to collect readings. Readings can also be entered manually to allow for use of non-Bluetooth-enabled devices.
      • Patient Assessments – Providers can gain qualitative and quantitative information from Patients using a fully configurable assessment tool.
      • Images – Patients have the ability to take and upload pictures to share with their care team.
    • Rich communications:
      • Secure messaging – Capable of synchronous and asynchronous messaging between the Patient and the care team. Patients can also request contact by phone or video telehealth visit.
      • Video telehealth visits – Providers can initiate video telehealth visits with individual patients or groups of patients.

      • Providers can direct patients to educational materials or other online resources
  • GTS SmartShare – Involving the family and friends of Patients has been proven to lead to better Patient outcomes.
    • Dynamic – Patients can invite family and friends to participate in the care process. Patients can choose how much or little to share and can change it at any time.
    • Drive treatment plan adherence – Where Patients share most of their interactions within the system, family members are key to helping drive treatment plan adherence.
  • GTS Provider Portal – Providers and the care team access the GTS-VP through a web portal.
    • Create and Manage treatment plans: Care team members can create and assign treatment plans for any disease state. Patients with multiple conditions may be assigned treatment plans by multiple doctors,
    • Drive Adherence – The GTS-VP has a series of tools that are intended to measure treatment plan adherence. These tools can also be used to help urge, nudge, cajole, etc. patients into changing their behavior and adhering to the treatment plans.
    • Exception-based Management – Provider efforts are enhanced by allowing them to focus on those patient who need the attention
